Relationship Between A1C and Fasting PlasmaGlucose in Dysglycemia or Type 2 Diabetes An analysis of baseline data from the ORIGIN trial

چکیده
OBJECTIVEdA1C measurement has advantages over measures of plasma glucose. Few studies have evaluated the A1C–fasting plasma glucose (FPG) relationship and whether oral antidiabetes drugs (OADs) and ethnic or geographic variations affect the relationship. Baseline A1C and FPG data from the Outcome Reduction with Initial Glargine Intervention (ORIGIN) trial participants were analyzed to 1) elucidate the relationship between A1C and FPG in people with moderate dysglycemia (A1C 5.6–9.0% [38–75 mmol/mol]) and additional risk factors for cardiovascular disease, 2) determine whether this relationship is altered by use of an OAD, and 3) study whether geographic and ethnic differences exist.

OBJECTIVE To assess the success and baseline predictors of maintaining glycemic control for up to 5 years of therapy using basal insulin glargine or standard glycemic care in people with dysglycemia treated with zero or one oral glucose-lowering agents. RESEARCH DESIGN AND METHODS Data from 12,537 participants in the Outcome Reduction with Initial Glargine Intervention (ORIGIN) trial were exa...

In their recently published Outcome Reduction with Initial Glargine Intervention (ORIGIN) trial (1), the investigators found that basal insulin glargine started early in the natural history of dysglycemia can maintain glycemic control near baseline levels for the whole study length. This trial is important for the size (12,537 individuals), geography (40 countries), and follow-up (up to 5 years...

Background: The aim of this study was to determine the level of agreement between the impaired fasting glucose (IFG) and abnormal glucose tolerance before and after application of the new IFG definition and to evaluate the impact of adding common clinical data on this agreement. Methods: A cross sectional population based study was carried out in an Iranian urban population which enrolled 8766...

W e thank Drs. Esposito and Giugliano for their interest in the glycemic control results from ORIGIN (Outcome Reduction with Initial Glargine Intervention) (1), and their recognition of the potential importance of our findings. Citing their own systematic reviews of literature, they point out that the ability of insulin treatment to attain good control of A1C levels is strongly related to the l...
